Community Health Best Practices, LLC

Founded in 2009, Community Health Best Practices, LLC is a nonprofit network comprised of fifteen of the nation’s leading community health centers.  Our Members operate federally qualified health centers (FQHCs) in Oregon, Washington, Arizona, California, Colorado, New Mexico, Texas, Hawaii, Connecticut, New York, Massachusetts, and Puerto Rico, and they provide comprehensive primary care and behavioral health services to all patients regardless of the patients’ ability to pay for such services.

Founded in 1996, Community Health Center Network (CHCN) is committed to excellent, affordable care for underserved communities of the East Bay. We are a non-profit Medi-Cal managed care organization, providing business administrative support for community health centers to focus on what matters most–patients. CHCN was formed to participate more effectively in newly-launched state managed care programs. The formation of CHCN built upon 20 years of collaboration in health policy and advocacy work through the Alameda Health Consortium.

CHIP, Inc. was founded in 1997 as the only statewide network of community health centers in the State of Maryland. As a leader and advocate for quality community health care in Maryland, CHIP provides community health centers with the business expertise to achieve the shared goal of quality improvement in the care they deliver.  In doing so, we give these centers access to higher levels of sophisticated health information technology (HIT) and management services than they could achieve and maintain individually.
