HITEQ Health Center Cybersecurity Defender Against the Dark WebIn recent years, health centers have become increasingly interested in and charged with not only addressing the health concerns of their patients, but centering and responding to patient’s social needs. Identifying and addressing unmet social needs as part of the clinical encounter provides the opportunity to deliver higher-quality, whole-person care, advance population health, and reduce healthcare costs.Despite recent momentum in the area of social needs screening, implementation at community health centers continues to be varied and uneven, and many are looking for guidance from peers on how to screen for social needs and respond to positive screens.

This badge is designed to support health centers by outlining promising practices for implementing their social need screening programs. To implement an integrated screening program that produces high-quality data, health centers must utilize digital health solutions and leverage their electronic health record (EHR). The resources in this badge share examples of these solutions in practice, and are designed to equip health centers with the information necessary to implement a screening program that limits burden on staff, is meaningful for patients and their care, and advances population health.

Lessons Learned: Implementing and Expanding Social Needs Screening Programs in Health Centers - Session 4: Level 4: Monitoring Population Level Data and Beyond

HITEQ Learning Collaborative Series

Is your health center currently in the process of considering, implementing, or revamping a social needs screening program within your EHR or health IT system? Join this learning collaborative to learn about health center promising practices and key considerations to support the successful collection, monitoring, and addressing of social needs data. During the series, participants will explore the levels of maturity in the social needs screening implementation process.

 

The levels of maturity include: 

  • Level 1: Coming to Consensus
  • Level 2: Implementing a Social Needs Screening Tool
  • Level 3: Responding to Positive Screens
  • Level 4: Monitoring and Using Data

Participants will gain information on concrete strategies and IT solutions that will help to improve internal systems, such as EHR utilization and care team workflows, and increase their capacity to advance individual and population-level health.  The HITEQ Center has partnered with the Louisiana Primary Care Association to design this series. Louisiana-based health centers will be showcased throughout the series to share their experiences with social needs screening, including successes, challenges, and lessons learned.

Session 4: Level 4: Monitoring Population Level Data and Beyond: Moving Beyond the Health Center's Four Walls
During this session, participants will explore what it means to move beyond simply collecting social needs data to participating in ongoing data monitoring and maintenance, and acting on data to  advance population-level health outcomes.

Topics: Monitoring data, Convening leadership to evaluate data, Data visualization, Data extraction, Importance of staff training, Improving service delivery, Establishing community partnerships, Emerging opportunities (risk stratification, advancing value-based care policy).
