This spreadsheet developed by the HITEQ Center provides a crosswalk of Clinical Quality Measures and their electronic specifications as defined in the 2022 update for Eligible Professionals (Clinicians). Fields include the crosswalk of measures with related information about CMS, NQF, and MIPS ID, and Telehealth Eligiblity, as well as inclusion in CY2022 UDS, Million Hearts, NCQA digital quality measures (dQMs), Quality Rating System Measure Set, CMS Adult / Child Medicaid Core Measures Set, and MSSP ACo Performance Pathway. Primary care plays a key role in ending the HIV epidemic. The Ending the HIV Epidemic initiative launched across HHS in 2019, setting forth four key strategies also called the “four pillars” of ending the HIV epidemic. Primary care in federally qualified health centers figures centrally into these efforts, evidenced by the addition of several HIV screening and prevention measures and metrics in the Uniform Data System (UDS) reporting. This webinar reviewed the outcomes of the first year of HIV Screening and Prevention reporting on the UDS, highlight opportunities for quality improvement, and discussed funding opportunities.

The coronavirus pandemic and consequent stay-at-home orders may increase danger for those at risk for or experiencing intimate partner violence and human trafficking (IPV/HT). Due to COVID-19, many health centers have shifted health encounters to virtual platforms, which offer unique opportunities to provide trauma-informed care and connect in new ways with those who may be experiencing abuse. Yet, telehealth and virtual visits also present health centers with new challenges related to privacy, safety and digital health equity. Given these changes in care delivery — and the inclusion of new Uniform Data System (UDS) data elements to capture IPV/HT diagnoses and services — health centers need information about how to identify and support patients at risk for or experiencing IPV/HT and leverage their health IT to provide and document care appropriately. In this webinar, presenters from the HITEQ Center and Futures Without Violence:

--Describe how health centers can implement an evidence-based, trauma-informed intervention for IPV/HT called CUES during virtual or in-person visits

--Review the newly included UDS data elements designed to capture IPV/HT diagnoses and services taking place within health centers

--Outline key considerations around privacy, safety, and equity for providing care through virtual platforms to patients at risk for or experiencing IPV/HT

--Feature promising strategies from health centers that have explored how to utilize health IT to support quality clinical care and data collection for IPV/HT

Community health centers have an important role in ending the HIV epidemic. In alignment with that important role, several UDS measures related to HIV screening, treatment, and prevention have been added for CY2020 UDS reporting.
 

This simple Excel file dashboard was shared by an existing FQHC and is used to depict performance on UDS measures over time.

This can be used to communicate with staff or monitor performance on UDS measures, and can be modified for use with other measures. There is also a tab with some real world advice on creating and using basic data monitoring dashboards included on the first tab.
