The 2018 Social Determinants of Health Academy

Virtual Training #4: APRIL: Pathways to Cross-Sector Partnerships to Address SDOH Part 2

The Social Determinants of Health (SDOH) Academy is a 6-month HRSA-funded virtual training series designed to help staff from health centers, health center controlled networks, and primary care associations develop, implement, and sustain SDOH interventions in their clinics and communities.

Designed and led by staff from fourteen organizations—all HRSA-designated training and technical assistance hubs—the coordinated curriculum exposes participants to a range of SDOH strategies and community-based interventions in a single place, and covers everything from screening to implementation and practice transformation. The Academy also provides an opportunity for health centers to engage in peer-learning and share their SDOH needs and work.

Each month from January through June, the Academy offers a virtual training on a different SDOH topic while also hosting corresponding virtual office hours where participants can ask questions and share their experiences. Building on the success of the 2017 pilot with 58 health centers and PCAs, the 2018 SDOH Academy is now open to staff from all health centers, health center controlled networks, and primary care associations.

The 2018 SDOH Academy will place a special focus on diabetes prevention and control, recognizing that improving uncontrolled diabetes requires a parallel focus on evidence-based clinical interventions and addressing SDOH. Where available, materials will also consider how SDOH interventions are funded, and what new partnerships or collaboratives may be necessary to finance work in the SDOH space.

2018 Participation Eligibility and Requirements

The SDOH Academy is open to staff and administrators at all health centers, health center controlled networks, and the primary care associations that support them. We encourage participants to join with multiple colleagues so you can discuss and digest the content together. These trainings are not open to staff from legal aid or community agencies that partner with health centers.

There is no formal application to participate in the SDOH Academy; simply register for the sessions you wish to participate in through the links below. You can choose to participate in some or all of the trainings. There is no cost to participate.

NOTE: All registrations will be pending approval from SDOH Academy staff to verify that participants are employed by a health center, health center controlled network, or primary care association.

Training Schedule and Registration

Each month, the Academy focuses on a different SDOH topic and offers both a formal webinar training, and a corresponding virtual office hours where participants can ask questions and share experiences related to that month’s topic. Please note: You can attend the webinar only for a topic, but you cannot attend a topic’s office hours unless you first attended the corresponding webinar.
