Resource Overview

Population Health Management requires aggregating patient data from a number of sources, and conducting analytics and modeling to derive actionable insights that translate to increased patient engagement and improved outcomes.  Resources in this section describe data sources that are available to health centers, how to access and integrate them, and ways to enrich them with patient-provided data through health risk assessments and patient engagement technologies.

Founded in 1972, Golden Valley Health Centers is dedicated to improving the health of our patients by providing quality, primary health care services to people in the communities we serve regardless of language, financial or cultural barriers. As a private non-profit Federally Qualified Health Center system they operate clinical and dental sites in Merced and Stanislaus counties.

Golden Valley Health Centers will be known as a premier organization ensuring access to high quality, culturally responsive and comprehensive primary health care for all, especially the underserved.
