Resource Overview

Population Health Management (PHM) is an evolving concept encompassing a suite of emerging technologies to aggregate, analyze and use data to improve clinical and financial outcomes.  PHM tools enable health centers to identify, monitor and target care to patients within a population. Resources in this section provide a conceptual foundation to help health center staff deepen their understanding of PHM and how the social determinants of health can be used to improve outcomes.

NACHC's Community Health Institute - Call for Poster Abstract Submission

Are you doing innovative things to address the social determinants of health or other topics of interest to health centers?  We encourage you to submit your work as a poster abstract to NACHC’s Community Health Institute (CHI)!  The CHI is the largest gathering of health center stakeholders on record.  This year’s conference will take place in Orlando at the Hyatt Regency Orlando from August 26th – 28th with the poster session taking place on Sunday, August 26th.

The deadline to submit poster abstracts is Friday, April 6th.

The NHIT Collaborative is convening the "Leveraging Health IT to Address Health Disparities: A Leadership Conference" in conjunction with HIMSS18.  The Leadership conference itself will be run from March 6 to March 7.

Join thought leaders representing multiple stakeholders at this two-day meeting to energize an agenda to improve the health of vulnerable populations through use of HIT.

National Association of Community Health Centers works with a network of state health center and primary care organizations to serve health centers in a variety of ways: 1) provide research-based advocacy for health centers and their clients; 2) educate the public about the mission and value of health centers; 3) train and provide technical assistance to health center staff and boards; and 4) develop alliances with private partners and key stakeholders to foster the delivery of primary health care services to communities in need.
