Overview

Through the data validation process, valuable information is gained that can then be used to improve performance around patient care or quality of care metrics.  Improving quality performance may take the form of improving data collection processes, better identifying patients who need additional interventions, or decreasing missed opportunities to provide patients appropriate interventions, among other possibilities. This section includes guidance on leveraging Health IT to improve quality performance including change packages for recommended approaches related to various quality of care measures.

A CMS Webinar

The Centers for Medicare & Medicaid Services Office of Minority Health and the Federal Office of Rural Health Policy at the Health Resources and Services Administration invite you to attend the “Connecting to Chronic Care Management Services” partnership webinar on Wednesday, March 15, 2017 from 3 p.m. to 4 p.m., ET. During this webinar, experts will provide an overview of the new Connected Care: The Chronic Care Management Services Resource initiative designed to educate health care professionals and consumers of the benefits of chronic care management (CCM) services. We would like to invite you and other representatives from your organization to participate in this event and consider partnering on the campaign. Professional, health system, patient, and caregiver organizations are encouraged to join this effort.
