Overview

Data monitoring, from the highest level down to the patient level is critical to identifying trends, gaining insights, and communicating transparently with staff and stakeholders. Data monitoring approaches such as dashboarding are used to display data in a simple and intuitive way, allowing a snapshot of performance on selected measures to see changes or areas for improvement. Business intelligence systems such as population health management analytics allows for the monitoring of the health of a whole patient population, stratified by various characteristics, thereby supporting care planning, resource allocation, and training opportunities. Resources in this section include tools to begin dashboarding, considerations for taking the next step with population health management and guidance on how to navigate the many factors of any data monitoring approach.

HealthHIV: TelePrEP in Practice

Webinar focused Readiness, Delivery, and Engagement

Designed to engage providers seeking knowledge and skills to implement the delivery of PrEP via telehealth (known as "telePrEP") in their practices, the session showcased models of practice and the role of clinicians and navigators within a PrEP care team. Speaker presentations were followed by a facilitated question and answer session.

Upon completion of this activity, participants were able to:

  • Describe how telehealth has transformed access to quality HIV prevention services.
  • Discuss aspects of provider readiness to deliver telePrEP, including the availability of telehealth technical resources for providers.
  • Identify opportunities and strategies to leverage existing resources and infrastructure development to support the extension/utilization of telehealth services for PrEP by the healthcare team.
  • Describe model practices for the delivery of effective and engaging telePrEP services, including hybrid (in-person and virtual) models.
