A NACHC Webinar Event

NACHC is excited to announce a series of Electronic Health Record (EHR) webinars to provide you the specifics of the EHR PRAPARE templates.  In these webinars, pilot testing teams of health centers and health center networks will talk about how they developed the EHR-specific PRAPARE template and the particulars in using the EHR template for data collection and reporting for use in care planning and management.  Health centers will discuss their experience implementing the EHR-specific template in their own setting and how they fit data collection of social determinants of health into their workflow.    

These EHR-specific webinars are meant to provide a “deeper dive” into PRAPARE and how it has been used on the ground.  For general information on PRAPARE and the tool, click here.
